- Assist Residence Manager in the overall operations of the IRA. - Assume responsibility for the facility in the absence of the Residence Manager. - Assist in the orientation, supervision and evaluation of DSP's and housekeeping staff. - Follow appropriate 624 Procedures relative to the reporting of incidents. - Schedule staff coverage and verify time worked. - Accompany residents on appointments and activities. Help teach residents skills for daily living and socialization and assist staff to do the same. - Ensure the implementation of habilitation Plans and daily activities and assist DSP's to do the same. - Purchase and maintain residents' clothing, personal items, adaptive and habilitation equipment. - Handle petty cash and maintain accurate receipts and account of expenditures as assigned. - Carry out fire drills, evacuations and emergency procedures. - Participate as a member of the Interdisciplinary Team in the development and evaluation of each resident's individualized Service Plan. - Assist the Manager with the maintenance and proper functioning of the physical plant, its furnishings and appliances and help obtain repairs/replacement as needed. - Review the log daily, report issues to Residence Manager for timely resolution. - Administer and verify distribution of medication. Use structured intervention techniques to help the employee learn to perform job tasks to the employer's specifications and to learn the interpersonal skills necessary to be accepted as a worker at the job site and in related community contacts.  - Can include related assessment, job development, counseling,advocacy, travel training and other services needed to maintain the employment - Will cater their role to working on improving the weaknesses and highlighting the strengths of the individual as outlined in the individual work plan. - Will familiarize themself with the goals the individual is working on and track the frequency of which he coaches the individual on those goals on the data collection sheet daily. - Will encourage the individual to advocate for him/herself, but can advocate on their behalf if that is absolutely necessary. - Will essentially learn all skills required to do the job. - When at a worksite, the job coach may have to prompt the individual on how to complete tasks, and if necessary model a task, but the job coach never completes tasks for the individual.   Residence Manager Position: - Supervise and instruct Lead Youth and Child Care Specialists (Lead YCCS) to ensure that the Youth and Child Care Specialist (YCCS) staff carry out their shift responsibilities. - Interpret and provide guidance around the agency's and programs policies to the clients, Leads as well as YCCS staff. - Serve as the Champion on the evidence-based model for Lead YCCS staff as well as YCCS staff and youth in the program to ensure all best practice programming is enforced as part of the model implementation. - Complete payroll sheets for the Lead Youth and Child Care Specialists. - Maintain accountability of petty cash and client accounts in adherence to budgetary guidelines. - Supervise staff to assist clients in developing good parenting skills and positive behavior while promoting independence. - Ensure that facilities meet compliance standards as required by NY Foundling, Administration for Children Services (ACS) and New York State (NYS). - Ensure that proper documentation is reviewed and submitted in a timely manner, i.e. (fire drills, progress notes on clients, shift logs and medication records, etc.). - Supervise and instruct Lead YCCS as well as YCCS staff to schedule recreational activities, shopping (food and clothing), laundry and prepare menus and food list on a weekly basis. - Provide weekly one-on-one and complete probation and annual evaluations for Lead YCCS. - Ensure that all Leads and Youth and Child Care Specialists receive all necessary training, i.e. (CPR, TCI, First Aid, Abuse and Prevention, Infection Control, etc.). - Ensure that the Lead YCCS or YCCS escort clients to medical appointments, ER visits and school, as needed. - Participate in all intake, weekly check-ins, ITM and treatment planning meetings. - Co-lead all monthly house meetings with other leadership staff. - Coordinate with all AOBH Program staff and AVP monthly staff meetings. - Ensure that all clients attend all structured programs, including tutoring, IL sessions, Health Group Parenting classes, Human Sexuality Sessions, etc. - Track reimbursable meals for the AOBH program and submit to finance. - Collaborate with the Nutritionist to maintain and edit weekly menus based on 4-week cycle. - Work with the Program Coordinator and Nutritionist to hold monthly wellness workshops highlighting various physical activity and nutrition related issues/topics relative to population at the AOBH. - Partner with the Nutritionist and Program Coordinator to plan cooking workshops to promote healthy eating and nutrition education at the AOBH. - Work closely with the Lead YCCS and YCCS staff to ensure compliance with Department of Health regulations for food service operations. - Collaborate with the Nutritionist to complete Food Protection Certification and train direct service staff in completion of Food Protection Course. - Elicit feedback from the residents on preferred food menu items. - Under the supervision of the Day Habilitation Manager of NYF Day Habilitation Services, the Day Habilitation Specialists is responsible for the following: Program: a. As a member of the leadership team, the Residence Manager is responsible for the overall supervision of the daily functioning of the Agency Operated Boarding Home (AOBH) of the young prenatal, parenting and postpartum youth. The Residence Manager should guide the Lead Youth and Child Care Specialists to encourage professional growth and to ensure policy alignment with the Youth and Childcare Specialist. The Residence Manager is expected to ensure that the Lead Youth and Child Care Specialist as well as the Youth and Child Care Specialist staff promote independent living skills and follow program processes and regulations. The Residence Manager will develop and maintain positive working relationships with all program staff, youth, stakeholders and all other entities. The Residence Manager will oversee all aspects of the operations for the AOBH residence that they supervise. The program has a bed capacity of 18, expecting/young mothers. The Residence Manager is responsible for building/maintaining community relationships, ensuring policy implementation, ensuring permanency, well-being and safety of all clients in the program. Job Responsibilities: - Supervise and instruct Lead Youth and Child Care Specialists (Lead YCCS) to ensure that the Youth and Child Care Specialist (YCCS) staff carry out their shift responsibilities. - Interpret and provide guidance around the agency's and programs policies to the clients, Leads as well as YCCS staff. - Serve as the Champion on the evidence-based model for Lead YCCS staff as well as YCCS staff and youth in the program to ensure all best practice programming is enforced as part of the model implementation. - Complete payroll sheets for the Lead Youth and Child Care Specialists. - Maintain accountability of petty cash and client accounts in adherence to budgetary guidelines. - Supervise staff to assist clients in developing good parenting skills and positive behavior while promoting independence. - Ensure that facilities meet compliance standards as required by NY Foundling, Administration for Children Services (ACS) and New York State (NYS). - Ensure that proper documentation is reviewed and submitted in a timely manner, i.e. (fire drills, progress notes on clients, shift logs and medication records, etc.). - Supervise and instruct Lead YCCS as well as YCCS staff to schedule recreational activities, shopping (food and clothing), laundry and prepare menus and food list on a weekly basis. - Provide weekly one-on-one and complete probation and annual evaluations for Lead YCCS. - Ensure that all Leads and Youth and Child Care Specialists receive all necessary training, i.e. (CPR, TCI, First Aid, Abuse and Prevention, Infection Control, etc.). - Ensure that the Lead YCCS or YCCS escort clients to medical appointments, ER visits and school, as needed. - Participate in all intake, weekly check-ins, ITM and treatment planning meetings. - Co-lead all monthly house meetings with other leadership staff. - Coordinate with all AOBH Program staff and AVP monthly staff meetings. - Ensure that all clients attend all structured programs, including tutoring, IL sessions, Health Group Parenting classes, Human Sexuality Sessions, etc. - Track reimbursable meals for the AOBH program and submit to finance. - Collaborate with the Nutritionist to maintain and edit weekly menus based on 4-week cycle. - Work with the Program Coordinator and Nutritionist to hold monthly wellness workshops highlighting various physical activity and nutrition related issues/topics relative to population at the AOBH. - Partner with the Nutritionist and Program Coordinator to plan cooking workshops to promote healthy eating and nutrition education at the AOBH. - Work closely with the Lead YCCS and YCCS staff to ensure compliance with Department of Health regulations for food service operations. - Collaborate with the Nutritionist to complete Food Protection Certification and train direct service staff in completion of Food Protection Course. - Elicit feedback from the residents on preferred food menu items. - Maintain and update the Local School Wellness Policy and Food Safety Plan accordingly. - Complete other job-related tasks, as required. - Certificate of Fitness for Fire and Non-Fire Emergency Drill Conductor F-07 must be obtained within the first 90 days of employment. - Certificate of Fitness for Supervision of Fire Alarm Systems S-95 must be obtained within the first 90 days of employment. - Perform other tasks as assigned by supervisor and/or administrator. The Fair Futures Coach is a full-time position that provides support and resources to help youth who have had experiences with the justice system, make progress towards their academic, career development, housing/independent living, and other life goals. The Fair Futures Coach will report directly to Fair Futures Coach Supervisor. The Fair Futures Coach will be responsible for:   Coach Responsibilities: - Build relationships with youth employing creative, out-of-the box, authentic, trauma-informed and strength-based approaches. Maintain a caseload of up to 18 youth. - Work 1:1 with youth to develop individualized academic, career development goals, and other life goals based on their interests and take measurable steps towards their goals. - Provide ongoing social/emotional support to youth. - Support youth explore their interests and expose them to opportunities in line with their interests. - Connect youth to specialists to help them identify and apply to best-fit schools/programs, career development experiences, and opportunities in line with their goals. - Help youth persist in academic/career development settings by checking on them weekly, visiting the school/program, forming a relationship with the primary person at the school/program. - Help youth navigate any challenges that arise and celebrate their successes. - Help youth plan/prepare for the next step on their journey. - Should a youth not be successful in any particular school/program setting, Coaches help youth reflect on the experience, provide support, and work with them to set new goals and transition to the next opportunity. - Collaborate with case planners, tutors, and specialists, as needed, to help youth achieve their goals and ensure their overall well-being. - Collaborate with parents, families, key adults, peers, stakeholders, etc. using a a strength-based approach. - Plan and facilitate groups related to academic support, life skills, etc. - Connect the youth to programs, resources, peer groups and opportunities in line with their goals. - Attend coaching and mentoring and group supervision with the supervisor at the office or in the community as needed. The Foster Care Case Planner works toward the goal of permanency for children either through reunification with parents, and if not possible, through adoption or preparation for independent living.  Trained and supported in implementation of multiple evidence-based models used to support families and caregivers.  Conducts birth home and foster home visits in addition to field visits; provide referrals to community service providers in accordance with the service plan for the family; accompanies families to drug treatment, medical, school, housing, welfare setting; works with children in onsite care programs; attends court hearings in a timely manner with prepared court reports and/or permanency reports.   Clinical Responsibilities: - Engage with birth parents, foster parents and foster youth using motivational interviewing techniques and assess for safety and risk factors for children currently in care and in planning for permanency. - Implement evidence-based and evidence-informed models with foster parents, birth parents/discharge resources, and youth. Under the supervision of the Day Habilitation Manager of NYF Day Habilitation Services, the Day Habilitation Specialists is responsible for the following: Program: a. Disability Specialist displays a congenial, warm acceptance for all consumers. b. They interact constantly with the consumers to develop a sense of security and belonging. b. S/he helps each consumer make the most of his stay in direct care. g. It is essential that Day Habilitation Specialist provide a solid program of constructive and interesting activities for the consumers. j. It is essential that Day Habilitation Specialist know and protect consumer's rights. k. Close supervision of the consumers is a vital part of the Day Habilitation Specialists' responsibilities. l. Finally, Day Habilitation Specialist must keep accurate records, pertaining to the day-to-day operation of the Day Habilitation program. Day Habilitation Specialist are required to have a high school diploma or GED and prior experience with OMRDD consumers preferred. Reporting Relationships: -Reports directly to Day Habilitation Manager or director for bi-weekly supervision. Education Requirement: -High School Diploma or equivalent with 5 years experience in the MR/DD field; or a Bachelors Degree in Psychology or related field and 1 year experience in the MR/DD field. -Responsible for the coordination, advocacy, follow-up, and administration of residents medical care appointments, logs, and medications. -Provide direct care and support for adults residing in our group homes. -Provide services in accordance with Residential Habilitation Plans, treatment plans, as well as The New York Foundling and OPWDD guidelines to develop skills required for a more independent lifestyle. -Coordinate, transport and attend all medical care appointments for individuals residing in assigned residence(s) in a safe and timely manner. -Communicate and advocate with medical care providers, nurses, clinical, management and program staff in regards to residents medical treatment plans. -Update all medical books/documentation as necessary and communicate all changes to appropriate persons as required. -Administer medications and document books/logs accurately and in a neat and timely manner. -Develop resident's basic living skills (e.g., social, domestic, and hygiene) through instruction and encouragement. -Assist and document the development and implementation of long and short-term goals for resident, as developed in the Habilitation Plan under the supervision or direction of the Program Manager. -Develop and maintain a positive, effective and professional relationship with residents, families, staff, administration, case management, and other service providers and assist in coordinating with any and all their needs concerning residents. -Perform or assist residents with personal hygiene as needed (e.g., bathing, brushing teeth, shaving, nail clipping, dressing, bodily functions, and service recipient appearance). Personal care services that are assigned by a health professional may include observation, reporting and documentation of changes in the status of the person or in body functions. This may include reading and recording the persons temperature, pulse and respirations and other needed personal care services. -Perform or assist residents in basic domestic duties as needed (e.g., cooking, cleaning, laundry, shopping). -Maintain records and complete paperwork as required by The New York Foundling and Office for People with Developmental Disabilities (OPWDD) (e.g., timesheets, mileage, petty cash, medication logs, resident's progress, visitor logs, Incident Reports and goal data). -Adhere to resident's behavior and health management plans (e.g., administration of medication, use of behavior modification techniques, dietary restrictions). -Maintain a safe environment for the residents; prevent harm to service recipient, self, and others. Report any safety concerns to management in a timely manner. -Manage residents and agency's funds in accordance with The New York Foundling policies and procedures. -Serve as a good role model to residents. -Maintain current certification as required by The New York Foundling and the residents Individual Service Plan.
