Use structured intervention techniques to help the employee learn to perform job tasks to the employer's specifications and to learn the interpersonal skills necessary to be accepted as a worker at the job site and in related community contacts.  - Can include related assessment, job development, counseling,advocacy, travel training and other services needed to maintain the employment - Will cater their role to working on improving the weaknesses and highlighting the strengths of the individual as outlined in the individual work plan. - Will familiarize themself with the goals the individual is working on and track the frequency of which he coaches the individual on those goals on the data collection sheet daily. - Will encourage the individual to advocate for him/herself, but can advocate on their behalf if that is absolutely necessary. - Will essentially learn all skills required to do the job. - When at a worksite, the job coach may have to prompt the individual on how to complete tasks, and if necessary model a task, but the job coach never completes tasks for the individual.   Residence Manager Position: - Supervise and instruct Lead Youth and Child Care Specialists (Lead YCCS) to ensure that the Youth and Child Care Specialist (YCCS) staff carry out their shift responsibilities. - Interpret and provide guidance around the agency's and programs policies to the clients, Leads as well as YCCS staff. - Serve as the Champion on the evidence-based model for Lead YCCS staff as well as YCCS staff and youth in the program to ensure all best practice programming is enforced as part of the model implementation. - Complete payroll sheets for the Lead Youth and Child Care Specialists. - Maintain accountability of petty cash and client accounts in adherence to budgetary guidelines. - Supervise staff to assist clients in developing good parenting skills and positive behavior while promoting independence. - Ensure that facilities meet compliance standards as required by NY Foundling, Administration for Children Services (ACS) and New York State (NYS). - Ensure that proper documentation is reviewed and submitted in a timely manner, i.e. (fire drills, progress notes on clients, shift logs and medication records, etc.). - Supervise and instruct Lead YCCS as well as YCCS staff to schedule recreational activities, shopping (food and clothing), laundry and prepare menus and food list on a weekly basis. - Provide weekly one-on-one and complete probation and annual evaluations for Lead YCCS. - Collaborate with the Nutritionist to maintain and edit weekly menus based on 4-week cycle. - Work with the Program Coordinator and Nutritionist to hold monthly wellness workshops highlighting various physical activity and nutrition related issues/topics relative to population at the AOBH. - Partner with the Nutritionist and Program Coordinator to plan cooking workshops to promote healthy eating and nutrition education at the AOBH. - Work closely with the Lead YCCS and YCCS staff to ensure compliance with Department of Health regulations for food service operations. - Collaborate with the Nutritionist to complete Food Protection Certification and train direct service staff in completion of Food Protection Course. - Elicit feedback from the residents on preferred food menu items. - Maintain and update the Local School Wellness Policy and Food Safety Plan accordingly.   Day Habitation Specialist Position: - Displays a congenial, warm acceptance for all individuals. - Interact constantly with the individuals to develop a sense of security and belonging. - Is responsible to support and supervise the individuals visiting the particular program assigned. - Interprets the Agency policies and program to the individual, other Day Habilitation Specialist and the community. - Is alert to the expressions of attitudes, feelings and relationships of each individual. Develops an awareness of the individual’s needs, not only through the observation and interaction of everyday living but, also through a study of the individual’s case history and by appropriate consultation with other members of the staff. - Must keep a record of pertinent observations and impressions about the group and the individual’s behavior. - Attempts to form a meaningful relationship with each individual in the group. S/he helps each individual make the most of his/her day at program. - Is responsible for matters of everyday living concerning personal cleanliness of the individual, and cleanliness of the program area, furniture, supplies and habilitation activities. - Provide a solid program of constructive and interesting activities for the individuals. - It is essential that Day Habilitation Specialist know and protect individual’s rights. - Maintain control of the individuals within the program in a discreet and non-threatening manner. - Must keep accurate records, pertaining to the day-to-day operation of the Day Habilitation program. Be sure to record and report any unusual or bizarre behaviors or incidents.   Interventionist Position: - Reports to assigned program supervisor or covering supervisor.  - Provides home-based clinical treatment utilizing the Functional Family Therapy – Therapeutic Case Management (FFT-TCM) model to assigned caseload of up to 10 cases and provides coverage for peer's caseload as needed   - Manages the extensive travel requirements involved in serving families in their homes.  - Conducts assessments including intakes, review of referral information, identifying and engaging key participants, identifying relational patterns, and individualized treatment planning.  - Engages families to sign for services and completes intakes with families within the required ACS timeframes.  - Participates in all required agency, model and stakeholder trainings, individual and group coaching and staff meetings.  - Adheres to model expectations and implement with fidelity including the timely completion of progress notes, session planning guides, session impact scales and appointment notes in the Care4 documentation system.   - Actively participate in weekly consultation and supervision and site visits when they occur.  - Identify and address family risk factors that give rise to possible child removal, ACS involvement, and out of home placement due to continued referral behavior or new risk/safety factors. This includes the completion of safety plans and incident reports if necessary.  - Identify community-based resources for families, collaborate and build relationships with stakeholders/providers (i.e., ACS, schools, doctors, mental health providers, courts, etc.) to monitor participation in services and accompany families to appointments if needed.  - Provides 24/7 coverage during the week and participates on a weekend on-call rotational schedule as applicable.     Youth and Child Care Specialist Position: -Supervise and support pregnant/parenting teens. Actively participate in effective implementation of recreation and structured programming for residents. Teach independent living skills (i.e. laundry, cleaning, cooking) and effective parents skills (positive role modeling); and co-facilitate groups when necessary; participate in the training and implementation of Trauma Systems Therapy and the successful dissemination of a strengths based behavior management system among residents, maintaining a thriving therapeutic milieu; - Provide safe child care and developmentally appropriate learning to call children in the program; - Change diapers, bathe, dress and feed children, as need or instructed. In the absence of the program cook, assist with meal preparation for residents; - Administer medications to the adolescent mothers, pregnant teens and children, as instructed; accurately document that medication on required forms; - Prepare room for new admission (sanitize and clean room, make bed with fresh linens, make room with welcoming basket), facilitate new admission orientation (review Resident M/C Program Manual), and ensure proper medical clearances; - Uphold expectations of the Primary role once/if selected and implement trauma informed responses with youth; -Assist the pregnant and parenting residents in developing effective parenting skills and positive behavior while promoting independence; - Comply with Agency rules, policies and procedure, as well as uphold Program standard operating procedures for Specialists and residents; - Work collaboratively with case planning team to ensure youths goals are identified and consistently monitored - Promote and maintain a safe, secure and nurturing environment for residents   Homefinder Position:  - Study/assess recruited and kinship homes, including gathering documentation, interviewing all family and household members, babysitters, and references. - Conduct home visits to assess the family dynamics and physical plant and safety of the home.
