The Foster Care Case Planner works toward the goal of permanency for children either through reunification with parents, and if not possible, through adoption or preparation for independent living. Trained and supported in implementation of multiple evidence-based models used to support families and caregivers. Conducts birth home and foster home visits in addition to field visits; provide referrals to community service providers in accordance with the service plan for the family; accompanies families to drug treatment, medical, school, housing, welfare setting; works with children in onsite care programs; attends court hearings in a timely manner with prepared court reports and/or permanency reports.
Position Description:
- Carries a caseload of up to 25 clients for linkage support
- Splits time between office and identified schools in assigned borough
- Research, organize, maintain, and disseminate community resource information to clinician in ongoing fashion on clients’ communities.
- Research, add information, and facilitate trainings and workshops for school staff, families, and youth along with the Clinician
- Works directly with clinician(s) as well as with school staff, managers, internal and with some interactions with clients as directed by supervisor or clinician.

The Enhanced Family Foster Care Supervisor supervises staff and cases in the enhanced family foster care program. Includes overall supervision and coaching of assigned case planners to ensure child welfare and administrative work as required by agency, city and state guidelines.
