Lead and optimize the Health Homes Care Management program to ensure high-quality, compliant, and family-centered service delivery. Drive strategic improvements in workflows, staff development, and program outcomes through data-informed decision-making and cross-functional collaboration.

Responsibilities

Lead and optimize the Health Homes Care Management program to ensure high-quality, compliant, and family-centered service delivery. Drive strategic improvements in workflows, staff development, and program outcomes through data-informed decision-making and cross-functional collaboration.

 

Responsibilities

  • Develop and refine workflows to strengthen referral pipelines and stakeholder engagement, with the goal of maintaining and increasing referrals through efficient processes.
  • Oversee referral assignment, outreach, enrollment, and case closure to ensure accurate weekly census reporting that supports data-driven operational planning.
  • Lead staff training and compliance initiatives to align with DOH and organizational standards, ensuring all staff are knowledgeable and compliant with program requirements.
  • Provide bi-monthly coaching to supervisors to support staff growth, case management quality, and leadership development through consistent feedback.
  • Prepare the departments for audits and maintain documentation accuracy that ensures data integrity and regulatory compliance.
  • Facilitate KPI review meetings to analyze performance data, identify trends, and implement action plans that enhance service delivery and outcomes.
  • Partner with leadership in recruitment and onboarding to maintain adequate staffing levels that support program success and operational goals.
  • Guide program billing reviews and revenue processes to ensure reimbursement accuracy and achieve financial performance targets.

Qualifications

  • Master’s degree in Social Work, Psychology, Mental Health Counseling, or a related field (required).
  • Strong knowledge of multi-systems in NYC, including healthcare, behavioral health, and social services.
  • 5 plus years of experience working with children and families affected by chronic medical and/or mental health conditions.
  • 3 plus years of supervisory and leadership experience, with the ability to mentor, motivate, and manage multidisciplinary teams.
  • Experience with field work and home visits, demonstrating understanding of direct service delivery and family engagement.
  • Strong time management and organizational skills, with the ability to manage multiple priorities in a fast-paced environment.
  • Care coordination experience with a focus on integrated and person-centered approaches.

Education Required

Masters’s Degree from a U.S. accredited educational institution or Credential Evaluation for a non-U.S. educational institution

Salary Range:

84,000-88,500
