Overview

At The New York Foundling, we trust in the power and potential of people, and we deliberately invest in proven practices. From bold beginnings in 1869, our New York-based nonprofit has supported hundreds of thousands of our neighbors on their own paths to stability, strength, and independence.  We help children and families navigate through and beyond foster care. We help families struggling with conflict and poverty grow strong. We help individuals with developmental disabilities live their best lives, and we help our neighbors access quality health and mental health services—core to building lifelong resilience and wellbeing. Together, our interrelated programs provide a whole-person, whole-family, and whole-life approach that unlocks solutions for a lifetime.

The Health Homes Care Manager is an incredibly important part of our team.  The Care Manager's role is to help children 0-21 years of age who struggle with complex medical, mental health, and/or psychosocial needs.  In this field-based position, Care Managers work closely with the child and their families to assess clinical, care coordination, access issues, and support needs, and to partner with families to develop a patient-centered Individualized Plan of Care. This plan helps to integrate all of the child's clinical and support providers, and to identify and problem-solve for unmet needs.

    • Monitor and evaluate a patient's needs, including prevention, wellness, medical, mental health, care transitions, and social and community resources where appropriate
    • Meet care management documentation requirements in a timely and accurate manner by effectively utilizing designated electronic record and communication management systems
    • Promote wellness and prevention by linking patients with resources and services, based on their individual needs and preferences.
    • Identifiy available community-based resources and actively manage referrals, access, engagement, follow-up, and coordination of services
    • Attend and participate in ongoing staff development trainings to enhance skills needed to effectively meet the demands of the Care Manager position
