Responsibilities

The Foster Care Case Planner works toward the goal of permanency for children either through reunification with parents, and if not possible, through adoption or preparation for independent living.  Trained and supported in implementation of multiple evidence-based models used to support families and caregivers.  Conducts birth home and foster home visits in addition to field visits; provide referrals to community service providers in accordance with the service plan for the family; accompanies families to drug treatment, medical, school, housing, welfare setting; works with children in onsite care programs; attends court hearings in a timely manner with prepared court reports and/or permanency reports.

 

Job Responsibilities

  • Maintain a caseload of 8 active and 2 suspended pay cases on average
  • Attend all court proceedings on time with prepared court reports and/or permanency reports. Be prepared to testify when required.
  • Conduct home visits to the homes of foster parents and birth parents / discharge resources.
  • Coordinate and facilitate family and sibling visitations/meetings. Supervise as required by the service plan
  • Provide extensive outreach/advocacy services. Engage absent or hard to reach family members and act as a resource for referrals and community services.
  • Any other duties as assigned

Salary Range:  $59,516 - $63,316

 

 

The Case Aide reports directly to Regional Administrative Manager for Foster Care Services. Functions as an active, co-participant with the Case Planning Team in providing services to Children and Families. The Case Aide supports the Case Planning team to promote and expediate permanency for our children and families. 


Job Responsibilities

  • Work as part of an interdisciplinary team to support and expediate the service delivery to our youth and families.
  • Provide support services to Families including advocacy, information sharing and resource referrals such as parenting skills, community MH services, HRA assistance, escorting to appointments, etc.
  • Conduct workshops for parents with focus on training them on the completion of housing applications, food stamp and health benefits applications, and other applications for services.
  • Responsible to assist Case planning team with driving agency vehicles to transport clients (children and families) to hospitals, residential settings, medical appointments, shopping emergency admissions and emergency replacement of clients etc.

** Valid Driver's License required. *

Salary Range:  $48,695.31 - $52,495.31

 

 

The Parent Advocate will support parents with children in foster care to understand and navigate the foster care system, make decisions for their families, remain hopeful, and address family challenges so that they can accomplish their goal of reunification. The focus of this job is building connections with parents and working collaboratively with case planners.

 

Required lived experience as a birth parent or a foster parent who had involvement with child welfare.

  • Collaborates with foster care staff on all issues related to the successful engagement of birth parents.
  • Provides direct support to birth parents including conducting home and community visits.
  • Prepares parents for and participates at Family Team Conferences.
  • Assists parents in utilizing community services, communicating effectively about their foster care case and meeting their service plan goals.
  • Informs parents of their rights and educates and advocates for parents in family court.
  • Assists parents in remaining connected to their children during foster care placement including supporting family time.

Salary Range:  $50,156.16- $53,956.16

 

 

The Home Finder will perform all steps necessary to train, assess and certify/recertify resource families. The Resource Parent Coordinator will be part of a centralized home finding department.

 

Job Responsibilities:

 

1.Study/assess recruited and kinship homes, including gathering documentation, interviewing all family and household members, babysitters, and references.
2. Conduct home visits to assess the family dynamics and physical plant and safety of the home. Ensure that all homes meet the physical standards required.
3. Interview collateral resources where necessary.
4. Write home studies that are both complete and timely.

 

Salary Range: $54,105.9-$57,905.90


Bilingual Family Trainer
the Family Trainer is responsible for conducting, managing and monitoring all foster parent Pre-Certification, i.e. MAPP; MINI-MAPP; TRIPP and on -going training for foster parents; i.e. KEEP and birth parents, i.e. 

 

Job Responsibilities: 

Send participants relevant materials, homework or any other supportive information to assist in their completion of the pre-certification training.

Work in conjunction with co-leaders and/or monitors to assess the readiness of the participants

Assist in writing up assessments of the participants with the co-leader and/or monitor.

Salary Range $57,680.00 - $64,980.00

 

Qualifications

Requirements:

  • BA/MA in Psychology, Sociology, BSW or MSW with 1-2 years child welfare experience preferred.

Other Requirements/Preferences:

  • Knowledge of community resources and ability to effectively negotiate systems for multi-problem families
  • Experience with drug treatment populations preferred
  • Bilingual Spanish/English required
  • Typing skills and computer literacy required
  • Excellent interpersonal skills
  • Valid Driver’s license

Education Required

Bachelor’s Degree from a U.S. accredited educational institution or Credential Evaluation for a non-U.S. educational institution
