Overview

The Family Resource Case Worker (FRCW) plays a critical role within the MST (Multisystemic Therapy) team by providing case management support to families. The FRCW engages with and assesses needs of families while collaborating with community-based programs (ex: schools, medical offices, etc) to ensure comprehensive support for families.

Responsibilities

  • Enhance Family Engagement: Provide clear explanations of MST therapy and its benefits to families, achieving high levels of family engagement and participation. Partner with Therapist to develop and implement specific interventions to overcome barriers family face with session pacing and work collaboratively towards treatment goals.

  • Maximize Resource Access for Families by becoming an expert on community resources and assist families in accessing these resources, including prosocial activities, public benefits, housing, legal, education, and medical services, leading to improved family stability. Collaborate with community resources to support family needs.
  • Maintain Accurate Case Records: Efficiently create, manage, and process cases in Evolv, Promis, Connections, and Ease, in a timely manner ensuring all contacts with families and collaterals are thoroughly documented, up-to-date and accurate.
  • Requests and obtain school, medical, dental or other applicable health records on an ongoing basis for all children under the age of 18 or relevant family members to ensure family's needs are being met and that there are up to date records on file to be in compliance with stakeholder regulations.
  • Provide Consistent Support and Advocacy for families by calling or completing in-person visits at least 1x per month or more depending on needs of the family. Attend meetings, appointments (e.g., psychiatric, school, HRA, housing) and conferences to be able to provide support and address any barriers to achieving successful outcomes.

 

Qualifications

  • Bachelor's degree in social work, Psychology, or a related field.
  • Experience in case management or social services, particularly with families and children OR Strong knowledge of community resources and ability to effectively collaborate with schools, medical providers, and other community programs.
  • A passion for serving children and families. Relevant experience in child welfare, social services, or a related field is preferred.
  • Excellent writing, communication, interpersonal, problem-solving, time management and organizational skills.

Education Required

Bachelor’s Degree from a U.S. accredited educational institution or Credential Evaluation for a non-U.S. educational institution
