The Community Care Coordinator plays a key role in supporting SCPS’s mission by enhancing access to mental health services through strategic community engagement, care coordination, and resource linkage. This role ensures continuity of care and improves family support - especially within the Identity & Acceptance/LGBTQIA+ Program.

 

Salary Range:  $53,500 - $57,000/year

Responsibilities

The Community Care Coordinator plays a key role in supporting SCPS’s mission by enhancing access to mental health services through strategic community engagement, care coordination, and resource linkage. This role ensures continuity of care and improves family support - especially within the Identity & Acceptance/LGBTQIA+ Program.

  • Represent SCPS at external community events and forums by serving as the primary liaison for potential community and business partnerships. Follow up with new contacts to establish formal collaborations.
  • Focus on growing awareness and referrals for the Identity & Acceptance program. Support development of at least 10 new active community partnerships per year.
  • Support families in outpatient clinics by identifying needs and linking them to external resources (housing, legal, insurance, etc.). Provide warm handoffs and application support to ensure families connect with services. Ensure at least 90% of families receive effective linkage to at least one support service.
  • Complete and submit referrals for families transitioning out of outpatient care. Coordinate with external providers, including C-SPOA, inpatient facilities, and substance use programs. Track referral outcomes to minimize gaps in service and ensure continuity.
  • Compile and regularly update a centralized database of resources (behavioral health, immigration, identity-based supports, schools, etc.). Share resources across all SCPS programs to enhance client support. Ensure the internal resource list is reviewed and updated quarterly.
  • Respond to all incoming program inquiries, including Building Futures campaign contacts, within 48 hours. Share program details and guide prospective clients through the referral process. Screen individuals for appropriate program fit, particularly for the Identity & Acceptance program.
  • Provide administrative support and coverage during staff absences to ensure uninterrupted services. Support front desk tasks, scheduling, and patient intake as needed.
  • Contact families to obtain any missing insurance details for clients in SCPS programs. Maintain accurate billing information for at least 80% of clients across School-Based and Community Clinic programs.

Salary Range:  $53,500 - $57,000/year

Qualifications

  • Bachelor’s degree in Social Work, Psychology, Sociology, Community Organizing or related subject area. 2+ years of care coordination/community organizing preferred.
  • Knowledge of community resources across NYC
  • Related work, lived experience, or meaningful interest in mental health, child welfare, culturally diverse populations, and event planning. LGBTQIA+ individuals strongly encouraged to apply.
  • Effective verbal and written skills
  • Excellent time management and organizational skills
  • Computer literacy; including Outlook, and Microsoft Applications (Word, Excel, PowerPoint, Teams)
  • An understanding and commitment to complying with the New York State Central Register of Abuse and Neglect protocol (requiring staff to comply with requirements to report any suspected abuse, maltreatment or neglect)
  • Demonstrated strong commitment to safety.

Education Required

Bachelor’s Degree from a U.S. accredited educational institution or Credential Evaluation for a non-U.S. educational institution
