The Care Navigator develops a POC for each family and works with their treatment team and the family directly to ensure all family members are connected to health insurance, a preventive health care provider, appropriate dental provider, and any other health provider that the family or treatment team deem necessary to help the family achieve their health and treatment goals. Work with their team to help coordinate the enrollee’s provision of services according to their health needs. Seek out and make referrals to preventive health care providers to address the family needs such as. Child wellness visits as per developmental guidelines, evaluations and follow up treatment for dental, vision and hearing care, following Medicaid EPSDT guidelines.
Monitor and evaluate a client’s needs, including prevention, wellness, medical, mental health, care transitions, and social and community services where appropriate.
