Case Planner is responsibile for clients in New York Foundling’s ESSHI Harlem young adult housing program. Housing young adults age 18-25, who have aged out of care and/or are experiencing chronic homelessness. Position reports to ESSHI Program Supervisor. Conducts home, community, and office visits; provide referrals to community service providers in accordance with the service plan for the participants; accompanies participants to drug treatment, medical, school, housing, human resource administration appointments as necessary. Role requires comprehensive case plan assessments and delivery of appropriate interventions. The Case Planner can expect to spend at least 50% of work time in the field or working remotely and must use tools and policies to record work time and maintain communication and accountability.
