Overview

Facilitator roles is to conduct all quarterly permanency-related Team Decision Making Conferences, Crisis driven conferences, and goal change conferences that focuses on children's safety and well-being

Responsibilities

implement conference facilitation model with ACS single protocol across the child welfare continuum with the goal of ensuring the shared lens of safety, permanency, well-being, stability, and equity are consistently aligned.


Engage with birth parents, foster parents, and youth as well as case planning staff, advocates and other's using motivational interviewing techniques while assessing for safety and risk factors for children currently in care and in planning for permanency. 

Utilize crisis intervention skills to deescalate conflicts in a trauma-informed approach

Send out electronic and paper invitations to all participants and document in connections


Maintain access to Connection and document the Service Plan Review within the timeframe as required by New York State, ACS and NYF compliance guidelines. 

Provide copies of FTC plan to all conference attendees at the end of each conference.

Conduct 2-3 conferences daily consisting of FASP driven permanency FTC's, Critical case conferences, including final discharges and certain goal change conferences. Alert management team when a safety concern is raised at an FTC immediately 

Collaborate with ACS by attending FTC regional debriefing meetings and site visits, as well as NYF regional meetings.

Computer Literacy 

Knowledge of child welfare policies and procedures

2+ years' experience 

 

Education Required

Bachelor’s Degree from a U.S. accredited educational institution or Credential Evaluation for a non-U.S. educational institution
