The case planner in the Mother/Child Residence is responsible for providing case planning/case management & social work services to pregnant and parenting adolescents who are residing in the maternity and mother/child residential programs, as well as providing after-care services to the parenting youth on trial discharge from current caseload.   Work collaboratively with all Program staff of each Residence; as well as ACS, the Referring Agency/Case Planning Agency for the pregnant teens, as well as the social work and medical staff of Bellvue Hospital Dept of Obstetric Care and Gynecology; Beth Israel Hospital; Sidney Hillman Institute of Family Health, and other Hospitals and Medical Clinic in the Bronx and Staten Island communities.

Responsibilities

Position Description: 

The case planner in the Mother/Child Residence is responsible for providing case planning/case management & social work services to pregnant and parenting adolescents who are residing in the maternity and mother/child residential programs, as well as providing after-care services to the parenting youth on trial discharge from current caseload.   Work collaboratively with all Program staff of each Residence; as well as ACS, the Referring Agency/Case Planning Agency for the pregnant teens, as well as the social work and medical staff of Bellvue Hospital Dept of Obstetric Care and Gynecology; Beth Israel Hospital; Sidney Hillman Institute of Family Health, and other Hospitals and Medical Clinic in the Bronx and Staten Island communities.

 

JOB RESPONSIBILITIES:

  • Provide case planning/case management and social work services to the pregnant and parenting youth in the maternity residence and the mother/child residential program;
  • Complete Intake interviews; psycho-social evaluation reports; collaborate and assist child care staff in developing ICMP (Individual Crisis Management Plan)
  • Collaborate with clinicians of NYF Mental Health Dept with regard to the youth’s initial mental health screenings and annual development screenings of the babies;
  • Collaborate with the respective Hospital Pre-Natal & Obstetric Clinic staff, and NYF Medical Clinic staff regarding the on-going health care of the pregnant and parenting youth and their infants
  • Provide weekly individual counseling sessions with pregnant youth and bi-weekly individual counseling sessions with parenting youth; fulfill the required contacts with the biological families of these youth; make home visits as required
  • Co-facilitate weekly therapeutic group with pregnant and parenting youth in the program
  • Document all contacts, interviews, casework and case planning sessions in Connections; Complete FASP’s and Court Reports/Permanency Hearing Reports in a timely manner.
  • Attend all required Court Hearings
  • Compliance with NYF, ACS and NYS face-to-face contacts and record-keeping requirements
  • Responsible for assuring the completion of all Mother/Baby Referral Packages for the pregnant youth in the maternity residence, which includes psycho-social evaluation, psychiatric evaluation and psychological evaluation for processing to ACS Office of Placement and the Case Planning agency
  • Attend bi-weekly team update meeting with the maternity and mother/child Program staff;
  • Responsible for scheduling; treatment team Conference for all pregnant youth with the case planning agency/ ACS/DCP, and the NYF psychiatrist and psychologist; schedule FTC conferences for the parenting youth;
  • Assure the implementation of the Service Plan Goals as agreed upon at the FTC conferences
  • Actively participate in individual and group supervision; clinical case conferences; treatment team meetings; FTC conferences; Discharge conferences; Program staff meetings and on-going In-Service trainings
  • Provide weekly parenting education group lessons to parenting teens in the mother/child AOBH program.
  • Provide weekly pregnancy education through individual and group lessons to pregnant teens at the AOBH program.
  • Attend all required trainings
  • Complete other duties as assigned

Qualifications

Requirements:

  • BA/BS in Psychology, Social Work or Sociology. MA/MSW preferred.
  • At least 2 years of Child Welfare experience, and experience in working with adolescents. 
  • Have a good understanding of adolescent development and pregnancy.
  • Good clinical skills.
  • Ability to communicate well orally and in written form.
  • Good computer skills and knows Connections.
  • Demonstrated strong commitment to safety.

Education Required

Bachelor’s Degree from a U.S. accredited educational institution or Credential Evaluation for a non-U.S. educational institution
