The Residential QIDP is responsible for the maintenance of assigned participants, development and implementation of Individual Service Plans. Supports and assists people in attaining their needs and goals through the most efficient and effective use of staff. Assures quality services by arranging for or providing staff training, developing and providing an environment that is conducive to the welfare of the people served and in accordance with federal, state, local and agency regulations and policies. Provides leadership and serves as an advocate to ensure appropriate treatment, teaching and protection of rights of the persons served. Works as part of the interdisciplinary team along with Day Habilitation Supervisors and clinical team. Recommends and assists with staff performance evaluations, and coaching. Develop and cultivate positive relationships with families and other personnel.
Facilitates the planning, development and implementation of person-centered plans; develops individuals goals and outcomes with people supported; coordinating and participating in individual plan meetings; monitors, documents and reports progress toward meeting outcomes; ensures supports needed are in place to meet outcomes; coordinates communications with other service agencies to ensure individual outcomes are met.
Evaluation of participant’s strengths needs and develop Habilitation Plans, designed to develop personal and life skills.
Ensure the timely receipt and filing of the Interdisciplinary Team summaries/ reports from various disciplines.
Ensure timely completion of summaries for monthly, semi-annual and annual reviews. Submit semi- annual and annual reviews to Service Coordinator for inclusion in the individualized Service Plan.
Complete addendum/changes to Residential Habilitation Plans as per team agreement.
Coordinate participants Residential Habilitation Services and collaborate with all clinical disciplines, which may include: Nursing, Speech/ Language, Nutrition , Psychology, Recreation, Medical, Pharmacy
Notify team members, parents, guardians, relatives and residences of all team meetings, reviews and apprise them of all habilitation changes affecting individuals.
Participate in the development of Individualized Service Plan (ISP).
Monitor and update all habilitation records per Interdisciplinary Team recommendations.
Ensure plans are always concurrent with valued outcomes outlined in the ISP
Develop, review, revise and monitor program goals (Valued Outcomes) as per consumer progress, needs and choices and in a timely manner.
Maintain Individual Goal Records, in a neat and orderly manner.
File and maintain participant progress notes, data records and meeting minutes.
Liaison with Schools, Day Program and outside service providers to ensure consumer services are integrated. Attend meeting with outside providers and other habilitation provides to ensure integration of services and accuracy in consumer’s strengths and needs.
Ensure Activity Schedule is consistent with the plan Activities.
Maintain a flexible schedule as per program needs
Monitor data sheets weekly
Accompany participants into the community to ensure community integration skill are being taught
Ensures opportunities to build social capital are planned, and occur; assist each individual in creating natural support networks.
Directs and coordinates the activities of all program, treatment and teaching of individuals in assigned caseload.
Oversees the development of integrated and coordinated active treatment program for each individual on their caseload.
Oversees/develops the Quality Assurance plans to monitor the implementation of each program to ensure Active Treatment.
Oversees/provides staff training and supervision to ensure that staff provide quality active treatment and personal cares according to regulations and The New York Foundling policy following established routines through coaching/mentoring/training.
Ensures timely completion of Client Coordinator responsibilities and paperwork.
Ensures contact with families, guardians and/or residences on pertinent matters and documentation thereof.
Serves on various committees: human rights, safety, high risk, ethics, incident report, etc.
Other duties as assigned

Salary Range: $58,870-$62,670/yearly
Bachelor’s Degree in Human Services or related field.
At least 3 years progressive experience working with individuals with ID/DD.
Have or obtain OPWDD-approved specialized training or experience in assessment techniques and behavioral support plan development.
