Responsibilities

The Foster Care Case Planner works toward the goal of permanency for children either through reunification with parents, and if not possible, through adoption or preparation for independent living. Trained and supported in implementation of multiple evidence-based models used to support families and caregivers.

 

Job Responsibilities:

  • Engage with birth parents, foster parents and foster youth using motivational interviewing techniques and assess for safety and risk factors for children currently in care and in planning for permanency.
  • Ensure that timely documentation is completed in accordance with the stakeholders regulations to provide the most up to date information for service planning and continuity of care.
  • Actively participate in individual and group coaching to promote professional development while strengthening team support.
  • Attend and participate in clinical interdisciplinary team meetings to foster collaboration across departments to provide the youth with preeminent services while working on permanency.
  • Completes referrals for services through NY Foundling and/or in the community to ensure continuity of care while in placement and on discharge.
  • Attend court hearings, court conferences and immigration appointments to enhance permanency outcomes and case progression.
  • Assure implementation of the service plan goals to provide the youth with support and casework counseling.
  • Maintain a caseload of 12 cases on average

Qualifications

  • Bachelor's Degree required in Psychology or Social Work, Master's Degree preferred 
  • 2+ years of Child Welfare experience
  • Experience working with adolescents
  • Excellent Communication Skills
  • Knowledge in adolescent development and pregnancy
  • Effective and efficient organization and time management skills
  • Good computer skills; able to use Connections
  • Strong interpersonal and team-oriented skills. strong engagement and motivation skills
  • Demonstrated strong commitment to safety.

Education Required

Bachelor’s Degree from a U.S. accredited educational institution or Credential Evaluation for a non-U.S. educational institution
