The Administrative Coordinator will work directly in the New York Foundling Health Centers in a client facing role. The Administrative Coordinator serves as primarily an administrative care liaison to our clients and their families, while providing administrative support to the mental health providers.

 

Salary Range: $62,000-$68,000 

Responsibilities

  1. Develop and maintain an understanding of Foundling programs to effectively coordinate and share information. Ensure seamless service delivery and improved outcomes for youth and families.
  2. Facilitate timely and effective coordination of initial and annual mental health assessments by supporting the medical administrative team, mental health skill builders, and regional AVP's, with a focus on improving client access to essential services. Support can include outreach efforts to families, case planning teams, and other programs.
  3. Ensure continuity of care by managing a roster of 29i youth that have transitioned out of NYF foster care. Coordinate linkage and referrals to external mental health providers including NYF A31 clinic. Serve as a liaison for clinical recommendations from psychiatry team.
  4. Act as primary liaison for the SCPS team for internal and external audits by supporting skill builders in obtaining necessary documentation for clients with community providers. Ensure required documentation is in EHR to maintain compliance with regulatory standards of external stakeholders.
  5. Promote integrated care by collaborating with the foster care case planning team, medical department, and mental health services to maintain a seamless continuum of care for clients across internal and external systems.
  6. Coordinate with community providers, hospitals and clinics to obtain medical records, evaluations and needed documentation after psychiatric ER/inpatient hospitalizations. Track receipt of this information and ensure compliance in NYF's Electronic Health Records.
  7. Ensure that consents and family planning letters are signed and uploaded by all parties into the EMR/EHR.
  8. Manage phone call reminders for initial psychiatric evaluations in the Bronx and Brooklyn. Support the psychiatric providers in coordinating recommended care with case planning teams.

Salary Range: $62,000-$68,000

Qualifications

  1. Bachelor’s degree in Social Work, Psychology, Sociology, Community Organizing or related subject area. 2+ years of care coordination/community organizing preferred.
  2. Strong computer literacy required; proficiency in using Electronic Health Record (EHR) systems to document services. Experience with myEvolv or Office Practicum a plus.
  3. Excellent communication and interpersonal skills
  4. Excellent critical thinking/ problem-solving skills
  5. Detail oriented and strong organizational skills
  6. Strong issue management and follow up skills
  7. Previous clinic or health center or experience in medical settings preferred.
  8. Bilingual proficiency in Spanish is a plus
  9. 2-3 years of Clinical Office Administration and Client Care Coordination experience

Education Required

Bachelor’s Degree from a U.S. accredited educational institution or Credential Evaluation for a non-U.S. educational institution
